How Admission Works

Call, email, or use the referral form below. Tell us about the prospective resident and their funding type.
Pre-Screening
We review the referral and conduct a brief pre-admission screening to confirm we can meet the resident's needs.
Home Visit
We invite the resident and/or family for an in-person tour and get-to-know-you meeting.
Move-In
Once approved, we coordinate the move-in date with the case manager, family, and discharge team.
Who We Serve
WeDoCare serves adults who need assistance with activities of daily living and benefit from a small, home-like setting with 24-hour staff support.
EW Waiver (Elderly Waiver)
Adults 65+ who qualify for nursing facility level of care. Billed through the resident's MCO.
CADI Waiver
Adults 55–64 with physical disabilities who need a residential alternative to a nursing facility.
DD Waiver
Adults 55 and older with developmental disabilities seeking a supported home setting.
Private Pay
$6,000–$7,000/month depending on level of ADL support needed. Contact us for a detailed rate sheet.
What to Include in a Referral
When contacting us about a potential placement, it helps to have:
- Resident name and date of birth
- Funding type (EW, CADI, DD, Private Pay)
- MCO / county case manager contact
- Current diagnosis and functional status
- ADL assistance needs (bathing, dressing, mobility)
- Medication list and any special medical needs
- Behavioral considerations (if any)
- Desired move-in timeline
